Yes. Please call our office for an appointment 913-642-0200 and press "1" for the appointment line. We have appointment schedulers available 24/7.  

You must sign a release of information and submit it to our office. We do have a form available online that you can print, complete and send in. The form is located under the registration drop down.

At least once a year or when you have changes to your information. This can be updated online or in the office.

Please bring insurance cards, photo ID, x-rays/diagnostic tests related to your condition, and a list of medications your are currently taking.


This is a contractual obligation between you and your insurance carrier.

Your co-pay is due at the time service is rendered.
