How do I get my medication refilled?
Contact your pharmacy or you may request refill on line.
How will I know my prescription has been filled?
Contact your pharmacy, if the request is denied we will contact you.
Can I get my prescription refilled on the weekend?
No, prescriptions are filled only during normal business hours
Are all of the doctors board certified?
Do I have airport/traveling restrictions?
No, if you have metal in your body you will probably be scanned at the airport, but you do not need any documentation from us for traveling.
Why do you need my social security number?
It is required by some insurance carriers as a component of verification.
If I am under the age of 18 can I attend the appointment alone?
No, you must be accompanied by a parent or guardian. You may have a letter from your parent/guardian authorizing another adult to accompany you.

How do I obtain my medical records?
You must sign a release of information and submit it to our office. We do have a form available online that you can print, complete and send in. The form is located under the registration drop down.
How often do I need to update my paperwork?
At least once a year or when you have changes to your information. This can be updated online or in the office.
What do I need to bring to my visit?
Insurance cards, photo ID, x-rays/diagnostic tests related to your condition, a list of medications your are currently taking
Why do I have to pay a co pay each visit?
This is a contractual obligation between you and your insurance carrier.
When is my co pay due?
At the time service is rendered.
