Why do you need my social security number?It is required by some insurance carriers as a component of verification. If I am under the age of 18 can I attend the appointment alone?No, you must be accompanied by a parent or guardian. You may have a letter from your parent/guardian authorizing another adult to accompany you. Are appointments required?Yes How do I obtain my medical records?You must sign a release of information and submit it to our office. We do have a form available online that you can print, complete and send in. The form is located under the registration drop down. How often do I need to update my paperwork?At least once a year or when you have changes to your information. This can be updated online or in the office. What do I need to bring to my visit?Insurance cards, photo ID, x-rays/diagnostic tests related to your condition, a list of medications your are currently taking Payment/BillingWhy do I have to pay a co pay each visit?This is a contractual obligation between you and your insurance carrier.
